Commission Payments
If you’ve worked in insurance before, you’re likely familiar with the hierarchy structures that exist within field marketing organizations (FMOs). At Essential Care, hierarchies are organized into levels one through nine, based on the number of agents above and below each agent. This structure can influence how commissions flow—something most common in Medicare sales but still relevant when selling ACA marketplace plans.
ACA commissions are paid monthly, with most carriers issuing payments early in the month. Keep in mind, commissions are only released once the member’s premium has been paid and successfully posted to their account for the prior month.
When to Expect Commission Payments
If you’re coming from other lines of insurance, you may notice the commission timeline for under-65 products looks a little different. Most carriers issue the first monthly commission one to two months after the policy’s effective date. However, a few factors can impact when you actually receive payment.

Late Premiums: If a client delays their binder or premium payment, your commission will also be delayed.

Unauthorized Plan Switches: If your client is switched into another plan or has their agent of record changed without authorization, you could lose commissions altogether. Always verify that your clients appear in your business book within your marketplace enrollment account.

Finally, keep in mind that CMS is actively working to combat marketplace fraud and implement safeguards to protect agents. In fact, unexpected changes in your commission payments may be one of the earliest warning signs that fraudulent activity has occurred.
What Factors Influence Under-65 Commissions?
Unlike Medicare, the ACA doesn’t set fixed guidelines for agent commissions. As a result, earnings can vary based on factors such as the state you sell in, the type of plan selected, and the number of household members enrolled. Understanding these variables can help you maximize the income potential of the under-65 market.
State Exchanges
Commission rates differ from state to state, with each exchange setting its own rules for what agents can earn. For example, one Florida carrier pays $30 per member, per month (PMPM)—a structure that can quickly add up when insuring families.
Since 2017, commissions in the ACA market have generally risen and remain competitive as the industry adapts to changing needs.
Per Member vs. Per Contract
The most common structure is per member, per month (PMPM). This means you receive a commission for every covered household member each month. Enrolling multiple family members magnifies your earnings.
Some carriers, however, pay per contract, per month (PCPM)—a flat monthly rate regardless of the number of individuals covered under that plan.
Agents earn initial commissions in the first year of enrollment, followed by renewal commissions in subsequent years. While many carriers pay the same rate for renewals, some do reduce the payout after year one.
Plan Type
The type of plan sold also affects commission amounts. Health Maintenance Organization (HMO), Preferred Provider Organization (PPO), and Exclusive Provider Organization (EPO) plans can each carry different commission rates. In some cases, carriers offer higher payouts for HMO or EPO plans.

To put the earning potential into perspective, here’s an example:
Jim, an independent agent in Illinois, earns about $20 per member, per month (PMPM) for each ACA policy he sells. When he enrolls a 43-year-old mother and her two children, that family generates $60 per month in commissions. Over the course of a year, that’s $720 from just one household.

OEP vs. SEP Commissions in the ACA Market
In the past, some carriers paid different commission rates for enrollments made during the Open Enrollment Period (OEP) compared to those made during Special Enrollment Periods (SEPs). That practice has since changed.
Starting in 2022, CMS required that commissions be the same for both OEP and SEP sales. The reasoning was simple: paying less for SEP enrollments discouraged agents from assisting clients outside of OEP, which CMS determined to be a discriminatory practice.
Today, most carriers offer equal compensation for new enrollments and renewals alike—creating consistent earning opportunities for agents year-round.
