Agents and agencies submit hundreds of Medicare enrollment applications each year. To keep your submissions moving smoothly through the system, we’ve compiled key tips for completing a Medicare application form accurately and efficiently.
Go Digital
Before diving into specific tips, let’s start with a big recommendation: transition to online Medicare applications.
As of September 2024, nearly 58 percent of adults have applied for Social Security online, highlighting the shift away from paper forms. With carriers improving their digital platforms, FMOs developing specialized tools, and a growing number of people over 65 online, submitting applications digitally is becoming the new standard. Leveraging technology to gather client information and complete Medicare sign-ups not only streamlines the process but also frees up time to focus on finding the best plans for your clients and boosting your lead generation.

1. Prepare Completely
Before starting a Medicare application, gather all essential client details—full name, contact information, date of birth, and Medicare Beneficiary Identifier (MBI)—in one convenient place. Interruptions that require navigating away from the application can lead to potential errors or loss of progress.

Implement reliable processes for collecting client information, such as a customer relationship management (CRM) system. Connecture and Sunfire also syncs seamlessly with your clients profiles, enabling clients to independently update their contact, health, and prescription information when convenient.
2. Ensure Clarity and Neatness
When filling out a paper Medicare application, always use blue or black ink and make your handwriting as clear as possible. Any ambiguity could prompt the carrier to reach out for clarification, which slows down the process.
If you can’t easily read the application, others probably can’t either. Reviewing the completed application with your client is a helpful step to confirm all information is accurate before submitting it.
3. Adhere to SOA Compliance Requirements
CMS regulations on Scopes of Appointment (SOAs) require agents to complete an SOA at least 48 hours before meeting with a beneficiary. Exceptions include cases where a beneficiary is within four days of a valid enrollment period’s end (such as AEP, OEP, SEP, or ICEP) or when a beneficiary initiates a walk-in visit.
SOAs are valid for 12 months, so be sure to obtain a new one if the original has expired. Tracking SOA start and end dates in your CRM is a best practice for staying compliant.
Important notes:
- You cannot complete SOAs for AEP before October 1.
- Ensure that at least one type of contract checked on the SOA matches the chosen plan on the application. Complete all fields legibly if using a paper SOA, as incomplete forms could lead to compliance issues.
- If additional plan types are to be discussed during the meeting, collect a new SOA for those products beforehand.
SOAs are valuable for future reference, providing easy access to clients’ contact information. Even when fields are labeled “optional,” it’s wise to complete every part of the SOA.
Per CMS rules, SOAs must be kept on file for 10 years, regardless of whether a sale occurred. While CMS guidelines may evolve, carriers may continue their own procedures, so it’s crucial to follow each carrier’s specific compliance requirements.
Filling out every field, even if marked as “optional,” is a proactive way to ensure thorough and compliant records.
4. Include All Required Details
When an applicant answers “yes” to any health-related questions that require follow-up information, make sure those details are provided on the application. Additionally, if the Special Enrollment Period (SEP) needs extra information—such as a chosen primary care provider, Medicaid, PDP, or Extra Help information—be sure it’s included.
Organize Details in the Client’s Profile
Beyond basic information, Connecture and Sunfire allows you to store comprehensive client details, including health conditions, providers, preferred pharmacies, and prescriptions. Alternatively, clients can add and manage these details using the client facing website PURL, which syncs directly with Connecture and Sunfire.
With everything stored in one place, you’ll have all the necessary information ready when completing an application through the CRMs.
5. Select a Primary Care Physician (PCP) When Required
If an applicant prefers a carrier to auto-assign their primary care physician (PCP), be sure to indicate this in the designated field. Some plans mandate a PCP selection, so leaving this blank can delay application approval.
For HMO plans in particular, applicants generally need to specify an in-network PCP. Double-check that this information is included on HMO applications to avoid any hold-ups during enrollment.
It’s always better to provide more information than less, so even if a PCP isn’t mandatory, encourage your client to list one if possible.

6. Ensure Attestation Matches the Election Period
The Attestation of Eligibility section must align with the chosen election period—whether it’s IEP, AEP, or SEP—in the Agent/Office Use part of the application.
If the election period requires a specific date (e.g., the date a client’s coverage ends), ensure it is accurately included. Additionally, for SEPs that need extra details—such as Medicaid, PDP, or Extra Help information—make sure these are provided on the application.
For any uncertainty about which election period to select, consult with your Essential Care sales specialist.
Service Area Reduction SEP
Clients whose Medicare Advantage or Part D plan will not be renewed for the upcoming plan year can qualify for the Service Area Reduction SEP, also known as SEP SAR. This SEP is available from December 8 through the end of February.
7. Never Modify a Client’s Signature
Regardless of intent, altering a client’s signature or the date on an application is strictly forbidden.
This rule applies to all related documents, including the SOA and any other forms accompanying the application, as well as your own signature or date as the agent. When in doubt, it’s better to start a new application with the client than to risk legal issues by altering documents.
Utilize Digital Signatures
Sending a prefilled application via email or text for a digital signature helps ensure the signature remains authentic. With digital signatures, the client controls the signing process, ensuring no unauthorized changes.
8. Ensure the Application is Complete
Each part of the application matters—to the carrier, your client, and to us. Carefully review the application to confirm that all sections are thoroughly completed before submission.
9. Submit the Application Promptly
After reviewing the application, submit it along with the necessary cover sheet and any additional required documents within the designated timeframe.
Applications must reach the carrier no later than 48 hours from the date of the agent’s signature. Late submissions may lead to penalties and delay both the enrollment and your commission—so submit on time to keep things moving smoothly.
