Every year, agents and agencies process hundreds of Medicare enrollment applications, and even small mistakes can cause big slowdowns.

To help your submissions move smoothly through the system, here are 10 practical tips for completing a Medicare application accurately and efficiently.

Paper applications are quickly becoming a thing of the past as carriers enhance their online platforms, FMOs introduce advanced tools, and more seniors embrace digital technology.

1. Prepare Carefully

Before starting any Medicare application, make sure you have all your client’s essential details ready — including their full name, contact information, date of birth, and Medicare Beneficiary Identifier (MBI). Having everything organized in one place helps you work efficiently and reduces the risk of losing your place or making errors if you need to step away mid-process.

2. Keep It Clear and Legible

When filling out a paper Medicare application, always use blue or black ink and take your time to write clearly and neatly. If any part of your handwriting is hard to read, the carrier will likely reach out for clarification — which can slow down the approval process.

Remember, if you can’t easily read your own writing, others probably can’t either. Before submitting, review the application together with your client to ensure every detail is accurate and easy to interpret.

3. Stay Compliant with Scope of Appointment (SOA) Requirements

According to CMS guidelines, agents must complete a Scope of Appointment (SOA) form at least 48 hours before meeting with a Medicare beneficiary.

There are a few exceptions when the meeting can happen sooner — specifically when the beneficiary has four days or less remaining in a valid enrollment period, such as:

  • Annual Enrollment Period (AEP)

  • Open Enrollment Period (OEP)

  • Initial Enrollment Period (IEP)

  • Special Enrollment Period (SEP)

  • Initial Coverage Election Period (ICEP)

  • Unscheduled, beneficiary-initiated walk-in appointments

Keep in mind that each SOA is valid for 12 months. If it expires, a new one must be obtained before meeting with the client. To stay organized, track the start and expiration dates in your CRM system.

⚠️ Important: SOAs for the AEP cannot be completed before October 1.

When filling out the SOA:

  • Make sure at least one plan type listed matches the plan submitted on the application.

  • Complete every field clearly and legibly if you’re using a paper form.

  • If you or your client decide to discuss additional plan types, a new SOA must be collected first.

Even though some fields may be marked “optional,” it’s best practice to fill out every section for completeness and future reference. The SOA can also serve as a quick way to access client contact details later.

Per CMS rules, you must retain all SOAs for 10 years, regardless of whether a sale was made. Keep in mind that individual carriers may enforce additional policies, so always follow both CMS and carrier-specific compliance procedures to stay fully compliant.

4. Include All Required Details

If a client answers “yes” to any health-related questions, make sure you add the necessary explanations or supporting details directly on the application. The same goes for Special Enrollment Period (SEP) submissions — include any extra information the carrier may require, such as the client’s primary care physician (PCP), Medicaid status, prescription drug plan (PDP) details, or Extra Help eligibility.

  • Medical conditions

  • Healthcare providers

  • Preferred pharmacies

  • Current prescriptions

Your clients can also manage this data themselves through their PURL (Personalized URL) Self-Service Application, which automatically syncs with both CRMs.

5. Choose a Primary Care Physician (PCP) When Required

If a client prefers the carrier to auto-assign a primary care physician (PCP), make sure to indicate that in the designated section of the application. Some plans require a PCP selection, and leaving that field blank can delay approval or trigger follow-ups from the carrier.

Most HMO plans specifically require members to select an in-network PCP. Always verify that the provider listed is within the plan’s network before submitting the application to avoid processing issues or rejections.

As a best practice, even if the plan doesn’t mandate it, encourage your clients to list their preferred PCP. Providing more information up front helps ensure a smoother and faster enrollment process.

Use Prefill and Copy Tools for Efficiency
As noted earlier, Sunfire and Connecture DRX can automatically store and prefill client information into new applications. However, not every field will populate automatically — so don’t overlook the simple but effective copy-and-paste method to keep applications accurate and consistent.

6. Align the Attestation with the Correct Election Period

Make sure the Attestation of Eligibility section matches the election period you select in the Agent/Office Use area of the application — whether it’s the Initial Enrollment Period (IEP), Annual Enrollment Period (AEP), or a Special Enrollment Period (SEP).

If the chosen election period requires specific details — such as the date a client is losing coverage — double-check that this information is included and accurate. Likewise, if the SEP calls for additional details like Medicaid status, PDP enrollment, or Extra Help eligibility, ensure that these fields are fully completed.

When in doubt about which election period to select, reach out to your Essential Care support team for clarification. Properly matching the attestation with the correct period helps prevent delays and ensures a smooth, compliant application process.

Service Area Reduction (SAR) Special Enrollment Period

If a beneficiary’s Medicare Advantage or Part D prescription drug plan will no longer be available in the upcoming plan year, they qualify for the Service Area Reduction Special Enrollment Period (SEP SAR).

This SEP allows affected individuals to enroll in a new plan or return to Original Medicare. The enrollment window for the Service Area Reduction SEP runs from December 8 through the end of February.

7. Never Modify a Client’s Signature

No matter the circumstance or intention, altering a client’s signature or the date on any Medicare application is strictly forbidden. This rule applies not only to enrollment forms but also to Scope of Appointment (SOA) forms and any supporting documents tied to the application.

The same standard applies to your own agent signature and date — both must remain accurate and unaltered. If a mistake occurs, it’s always better to start a new application with the client rather than risk forging or falsifying a legal document, which can lead to serious compliance violations.

Use Digital Signatures for Added Security
To help prevent errors or unauthorized changes, consider emailing or texting a prefilled application to your client for digital signing. This method ensures the client maintains full control over when and how they sign — eliminating the possibility of signature tampering and keeping the process secure and compliant.

8. Ensure the Application Is Fully Completed

Every section of a Medicare application matters — for you, the carrier, and most importantly, your client. Incomplete or missing information can cause delays, rejections, or extra follow-ups that slow the enrollment process.

Before submitting, take the time to review and double-check every field to confirm the application is thorough, accurate, and complete. A careful final review helps ensure a smooth approval process and protects your client’s coverage timeline.

9. Submit Applications Promptly

After carefully reviewing the application, make sure to submit it promptly along with the required cover sheet and any supporting documents within the carrier’s specified timeframe.

Applications must be received by the carrier no later than 48 hours after the agent’s signature date. Submitting late could lead to enrollment penalties from the carrier — and it also delays your client’s coverage and your commission.

In short, timely submission keeps everything moving smoothly — ensuring your clients get enrolled quickly and you get paid without unnecessary delays.
